HealthPartners Living Well Program (Required for health insurance deductible discount)
HealthPartners has updated their wellness/wellbeing portal and it is now called HealthPartners Living Well. This is the portal used when you complete your health assessment and wellness activities to earn your preferred deductible for the 2024-2025 plan year. If you need help logging in to an existing account or creating a new account, please see these instructions.
What is the preferred deductible?
The preferred deductible is an optional benefit offered by the district. If you complete the required health assessment and wellness activities, you will receive a $500 deductible discount with a single plan and a $1,000 deductible discount with a family plan for the 2024-2025 plan year.
How do I earn the preferred deductible?
You must complete the HealthPartners online health assessment and two of the provided wellness activities.
NOTE: If you are a new employee or new to the plan this year, you were grandfathered in with the preferred deductible for the 2023-2024 school year. To keep the lower deductible for 2024-2025, you must complete the wellness program outlined in this email.
What is the deadline?
The deadline for completing the requirements for the preferred deductible is March 31, 2024.
How do I get started?
1. Go to the new HealthPartners Living Well link: www.healthpartners.com/signin/livingwell. Access your existing account or set up a new account if you don’t already have one.
2. Complete the health assessment.
3. Complete two activities from the list provided.
- An annual exam between April 1, 2023 and March 31, 2024 with your healthcare provider counts as an activity. If you choose this option, have your doctor sign the attached Annual Physical Exam Form and return it to me or Twila in the District Office.
- Some activities take several weeks, so don’t wait until the last minute – go check out your options now so you make the deadline.
4. When you have completed all requirements, the living well page of your portal will indicate that you have earned your benefit.
NOTE: If you complete the annual exam as an activity, it may take up to 14 days for the completion to show up in your HealthPartners account due to reporting and site updates between our office and HealthPartners.
